Understanding home health care

Patients need to be under the care of a doctor, and the home health plan is coordinated with that physician with approval from Medicare/Medicaid or waiver program.
Medicare generally does not cover non-skilled or custodial home care, such as personal care, homemaking, or companionship, when these are the only services needed; however, eligible individuals may have coverage through Michigan Medicaid, waiver programs, or other insurance benefits.
The duration and schedule of Home Health Services vary for each patient based on their individual needs, physican- or practitioner-approved plan of care, and applicable insurance coverage and authorization. Services are provided according to the authorized plan of care and may be adjusted as the patient's needs and condition change.
For individuals receiving services through a Medicaid waiver program, the number of approved service hours is determined by the individual's needs assessment and authorized service plan. National Home Health Service provides services based on the hours and service authorized by the waiver or service plan.
No. Services must be provided according to the approved plan of care or authorized service plan. If additional hours are needed, the client or authorized representative should contact the appropriate waiver service coordinator or insurance plan to request a reassessment or review.
